Welcome to the next post in our series of Product Snapshots in the Australian healthtech sector.
Say hello to Chroniq Health.
In one sentence, describe your product?
Whole of practice chronic disease management platform that goes beyond "ticking the boxes" to improve health outcomes.
What industry challenge is Chroniq Health tackling?
GP, Nurses and Patients are wanting real change to health outcomes but are operating within outdated technology platforms.
COVID has highlighted the need for a chronic disease management platform that is relevant, patient centred and outcomes driven as we continue to deliver care an ever challenging environment.
What makes Chroniq Health unique in the market?
There are a number of unique differences, but the one we are most proud of is that the platform was designed with real life workflows in mind.
We understand that chronic disease management is shared between the Patient, the Nurse and the GP, all playing an important role.
The patient is at the centre, with goals developed to support their individual care needs. The nurse has the flexibility to develop a care plan that reduces unnecessary time "ticking boxes" with more time to "be" with the patient, and the GP has a care plan that is patient focused, relevant, and Medicare compliant.
What is the biggest risk to the company?
Medicare funding is the ever present risk to general practice, and is a risk shared by many.
However, we feel confident that chronic disease management will be a continuing focus for Medicare and by developing a platform that is outcomes driven we will be well positioned to offer care well into the future.
What does success look like for Chroniq Health?
Success will be hearing how Chroniq Health has improved health outcomes, hearing nurses say they are enjoying care planning and connecting with patients, and hearing GPs say chronic disease management has taken on a new life at their practices.
Why should a customer use Chroniq Health?
Chroniq Health will allow practice to deliver the chronic disease management services they have always wanted to, patient centred, outcomes driven, and rewarding.
